I only needed my passport, retirement visa, rental agreement, and TM30 to get my Bangkok Bank account in Bangkok. Nothing mentioned about insurance. I did have my Thai wife with me who did most of the talking.

The agent I used for my first one was 1500 baht. 300 baht is a steal. Let them do the first then do it online after that.
