 no I did not, but you can always ask the hotel and they will give it to you no problem

 I have applied for 2 Thai passes with Agoda hotel and both times my application was accepted.

A friend and myself have booked with agoda and as long as it says airport transfer and pcr tests then they will accept. But after booking on agoda you can also contact the hotel directly and ask for a receipt for quarantine application.
