TR visa online. Just Google thai e-visa. I did mine few weeks ago it was only 2 days to get mine. And you can extend for extra 30 days for 1900 baht in any immigration office

Bike licence 105, doctor certificate 200 baht, proof where you live certificate (i don't remember the official name) 300 baht. I paid this in February when i did my bike licence. 605 baht in total.
