AXA did attempt to reimburse me but they tried to reimburse a credit card which isn't going to work with a Swift transfer. The issue really is I can't get anyone at AXA to respond to me to fix this. I just emailed AXA's bank (Citi) to contact me to help resolve this so we'll see what happens and I already got an automatic reply that they'll be contacting me soon.

Got the release papers. Now the question is do I need another test 72 hours prior to traveling to Koh Pangan?

I wasn't sure what exactly they wanted, since the US passport already uploaded should prove citizenship, so I just uploaded my US birth certificate for that one.

I am coming from the US to Samui in October on a TR Visa and the Thai Embassy in Los Angeles asked me to provide them with my outbound flight information. Makes sense because the TR is a tourist visa. If I had set up a retirement visa in the US I'm pretty sure that they would not require a return flight.

That's odd because by the time I was getting back into my car at USPS I got the email already. Check your email spam folder maybe? But you should have your hard copy delivered by mail within the next few days.

I got an electric copy emailed to me almost immediately after I did the fingerprints at USPS and then a hard copy of it came in the mail about a week later. If you got the electronic copy you can always upload that document onto the Thai embassy's website in your visa application. You don't have to wait for the hard copy to come in the mail.
