 it's for anyone who enters Thailand. It's an entry card. It's nothing to do with visas. Irrelevant whether you're on visa or not, if you enter you fill it in.

I boarded in UK. Put UK. I transmitted in Delhi airport but didn't enter the country, so didn't used that. I used the UK as departing country.
