Make sure you only ask for it once the year extension is inked in, not if there is an under consideration stamp etc.

Show your entry stamp and visa into Vietnam and then your domestic flight boarding pass etc to Hanoi. It's not rocket science.

Do I need to obtain re-entry visas with a multiple-entry 90 Day TR visa before leaving Thailand?

There is no 90 day multiple entry visa. There is a 6 month METV visa granting you multiple entries, each entry is 60 days and can be extended once per entry another 30 days. You need to enter before the expiry/enter before date on the visa for your final entry and stamp.

Do I need to write the explanatory document myself for the Thailand Destination Visa, or does the company provide it?

Anonymous participant just write the short letter specifying which DTV you want and why the work you do qualifies for that DTV. Nobody is asking for a signed, sealed, stamped, letterhead certificate.
