Jef from what I understand, you can only apply for the pay & tour card at the KBank exchange at Suvarnabhumi airport in Bangkok. If you know of somewhere else, please reply. Thanks.
I just got my DTV as a remote worker (search this group for my list) and I included a simple profit & loss statement from Jan 1-current, copies of business bank statements and a remote worker policy. Template is below:

I had tried to upload samples of my documents to the Files sections of this group but Admins requested I remove them due to Facebook restrictions that might jeopardize the whole group.
My husband and I applied for the LTR remote worker visa (qualified in all aspects) but after repeatedly being requested to resubmit the same information multiple times we have finally given up. The BOI staff was helpful but just could not understand unlimited private international health insurance (even with a policy summary and highlights) and then they started requesting the financial statements of our NYSE employer. We could never get past customer service in our numerous calls to them. Eventually, after a couple months, the remaining months on the health insurance policy became too low which would have required a purchase of a separate policy. Also, the requirement to reapply and pay another 100,000B (2 people) when we switched from remote work to wealthy pensioner in 5 years wasn’t worth it.
This just happened to me in Oct 2023 by air into CMX. Came in on a 30 day visa exemption and they stamped it for 90 days. Got it corrected but didn’t catch that it was now stamped to expire Nov 2024 until I went to Immigration to apply for a 30 day extension. Luckily Immigration took care of calling the airport and changed it in the computer and my passport.
