It also depends on your nationality and willingness of your embassy to support your stay here with a letter saying you cant travel. Otherwise it's border bounce, but they are closing fast or becoming complicated.

I'm guessing, and it's a guess please don't shoot me. That the thai address given on the OP's paperwork matches with that already provided by the hotel owner, so no questions. It may not be the case that they don't want tm30 info, they already have it.
