I opened one with Bangkok bank. They have a form and will tell you everything needed. Just ask them not FB.

Lufthansa.. I tried but we they wouldn't let us board with our current tickets. Trust me I was pissed.

Unless you have a multiple entry visa, when you get to the counter at check in you will have to change your ticket to leave Thailand in less then 30 days. I just went through this. Expensive fees plus cost of new ticket.
