well you have to show that TM 30 form because as I remember they ask for tm30 and i have to go another day there with the form, i dont know about online but in person you have to bring that with you

yea! I was surprised, during my report days I have to go there 3 days in a rows because of lack of information & documents and each day I have seen queue ticket number exceed 600
