I believe it's 8 weeks between doses of AstraZeneca in Thailand. 21 days is only the interval for Pfizer. If you don't have the extra time I would forego the AZ and look for Pfizer or Moderna. You can actually purchase Moderna from third parties (even on lazada and shoppee, or contact hospital and ask).

It may cost more but you might find that your timeframe can be shortened considerably.

You can get vaccinated in Thailand. You will not find Johnson and Johnson in Thailand (one shot only). You should be able to find AstraZeneca or Pfizer. You can complete your two-dose series faster with Pfizer (less wait time between doses) than AstraZeneca although AZ is probably easier to find. You then have to wait for a second dose. You then have to wait for 14 days after the second dose in order to board a plane.

In other words, you need to work on getting vaccinated now. Try to get two Pfizer shots if possible. Generally, if you are accepted for your first dose somewhere you will be given an appointment at the same location for your second dose. In the meantime, you should be able to continue on covid extensions (you actually have an excellent reason because unvaccinated you can't return to your country of origin). You need to make getting vaccinated your first priority.

There are two types of health insurance needed for Thailand: for entry and for extension of stay (only a Non-OA for the latter). All non-Thais must have $50k USD coverage for the period of time they get for their initial permission of stay. So enter on a visa-exempt entry get 30 day permission to stay, need 30 day insurance coverage. Enter on tourist visa get 60 day permission to stay need 60 day insurance. Enter on 90 day Non-O, need 90 day insurance coverage. Enter on Non-OA get 1 year need 1 year insurance. Enter on re-entry permit of Non-O visa get stamped in for time until extension expires. Despite this rule there have been some reports that only 30 days insurance is needed for entry or to get a Thailand Pass.

Then when you extend a Non-O you don't need insurance, when you extend a Non-OA you do. The coverage is for 400k baht inpatient and 40k outpatient. This is scheduled to increase to 3 million baht coverage by October 2022.

Can get copies and photos made at shop on grounds of phuket immigration for about 2 baht/copy.

Sounds like you paid a convenience fee for having it done inside the immigration office without losing your place in line and going to the inexpensive copy shop outside.

It took about 24 hours for my application for an account was approved. Then signed on, filed the report and received approval later that same day. I think the speed of the process depends on how busy your immigration office is as I think they manually approve things. Good luck.

yes, correct. Although no harm tying the new system even the first time.

The previous system needed you to enter the TM-6 arrival/departure card number. Of course that number changed with every entry. The new system doesn't ask for that number so worth a try even with a new entry.

You have to apply to get an account, one time only, thereafter login to you account and file the 90 day report. It's a new system as of December. It worked for me a few weeks ago. Filed report was approved the same day.
