I went to the U.S. Embassy in Bangkok a couple of weeks ago. I have my certified copy of Marriage Affidavit and passport. I'm now getting the affidavit translated for 500 baht.

Go to your local amphur and ask them for a list of what you need.
We did that a few weeks ago. All that I needed to get was a document that says that I'm not married. Had to go the the U.S. Embassy in Bangkok to do that.
