I'm applying for a new extension of stay. Entered before 21 Nov 2013. Visa due to expire end of October just before I leave Thailand for 3 months. This is my 4th year here but I'm having trouble finding exactly the renewal requirements as it changes.

What are the latest requirements for renewal of Non Immigrant O-A Visa and can I download the form? Quite confusing the websites.

Because I needed a Fitness Certificate for Australia as well (because I have a minibus licence) total cost was 940Bht. Cost of each licence 510bht.

I did both last week. Stat dec for residency from Australian Embassy plus medical certificate from Doctor at Phaya Thai Hospital. Stat dec from Embassy was around 1800bht. Also copies from passport required. Because I had both licences from Australia only eye tests at Dept Land Transport required. No driving test required. Copies you can get there.
