maybe as you say I arrived last Tuesday and they had people prior to immigration just scanning the Thai pass I don't recall them Checking insurance. So if the Thai pass is OK with just 30day cover then I'll save next time when I return for 3 months.

from what I understand your Covid insurance needs to cover the period of your stay so 60 days or you may have problems at immigration or having your application for Thai pass rejected.

What advice can you give for booking an SHA+ Plus hotel directly in Thailand with required PCR testing?

The way I did it was to get my friend in Thailand to ring the hotel and book it directly she also checked to make sure it was a good deal with included testing and pickup. She paid on my behalf and the hotel emailed me confirmation.

Well unfortunately you don't have a choice. The hotels have a partner Cotract with a specific hospital. Also it's advisable to read the fine print on your policy.

Do you have to isolate in Thailand if you sit next to someone who tests positive on a flight?

If you are seated right next to an infected person I'm afraid you will thrown into isolation as well. That's the risk you take traveling to Thailand.
