What are my options to extend a 60-day METV in Chiang Mai, and how is the validity calculated?

I'm in Chiang Mai on a 60 METV issued to me on 10/10. My visa stamp says I'm admitted until 12/22. What is the best way to extend my visa for another 30 days? Should I extend at the immigration office, or plan my trip to Laos sometime before the 22nd? I have a US passport.

Also – I just want to confirm that the visa is valid according to the stamp date I entered Thailand (10/24), not the date of issue for the visa (10/10)? Thank you
