If you in fact have an O-A visa, you will not get another year by leaving Thailand. But you will get another year if you RETURN to Thailand before the visa expires

I guess the visa you want to apply for is a non-immigrant O visa, based on being over 50.

In Vientiane they do require a doctor's certificate and a police clearance.

But I'm not sure about their requirements though

Most embassys don't require you to show money for a non-immigrant O visa, based on marriage to a Thai national.

However, if you want to apply inside Thailand, and for subsequent extensions of stay,

you need a Thai bank account in YOUR name showing at least ฿400 000 OR a monthly income in your name of at least ฿40 000.

You can not use a combination of income and banked money for this type of visa/extension.
