You need to have at least 15 days left of your admitted stay when you apply

You can rule out O-X, as that wasn't a thing in 2010.

Whether you had an O or O-A visa can only be determined by looking at the actual visa you started out with.

Your picture shows no visa, but an extension of stay and a re-entry permit
