Your passport (with various copies), 1 picture, house book or rental agreement, proof of income or banked money and application (TM7) is what you need in Chonburi.

But as I said, this may vary in other provinces

If that visa is a valid multiple entry visa, you can leave Thailand and return for a new stamp.

But this is not the same as extension of stay
