I believe it's 1000thb, definitely at Phuket town immigration. I belive also at airport but I've never done it.

TM30 notification of address.

Re passport no experience or knowledge on that, sorry

Re leaving country, yes but you will need a re-entry permit to keep the visa live. Initial 90 days then can be extended for one year anytime after 45 days

this list is from Phuket immigration yesterday. I cannot stress enough, go and speak to the right person at your local immigration office. I only got this list after 3 visits giving conflicting information.
