No need to be married, yes your name on certificate, they also put my passport number on it as well.

CM was the same last time I went, old bag who was just bloody awkward, that's why I went onto a Non O ME so that I didn't have to face it again, I have a problem dealing with idiots and unfortunately I tell them, not a good idea when you need a visa from them!😉

As you say,those are required for the annual extension but not for the 60 day extension. My stamp is good till 26th November. They didn't mention the TM 30 but I do that online, so I presume it's in the system.

Yes, it was. Best experience I've had, 2 years ago I got a yearly extension by an old bag who made out she couldn't speak English and she was bloody awkward through the whole process, that's why I did a Non O ME last December.
