They do charge $50 to notarize the passport. If you'd the the phone number for the guy that had the passport translated at the MFA I'd be more than happy to PM it to you. BTW, what Amphur are you going to? Just curious.

There is no form for the notary concerning passport certifications. They will ask you what it is for and as long as it is not for marriage notarize it. I'm not sure of the difference but that's what they ask.

FYI....each Amphur has somewhat different requirements.

Notarized by the US Embassy, translated to Thai and then certified by the Ministry of Foreign Affairs. I had it translated and then translated again by a place at the MFA. They called on of the girls upstairs that actually certifies the documents and she translated it to exactly what they would certify. If you go to the MFA website they give you the format and examples. In addition to my personal info of the picture page her translation included the Embassy's stamp translated and the constitution preamble which was on the example on the MFA site. I think if you didn't include that page in your copy they wouldn't need it translated though. Scan it and then crop the only page you need perhaps? Good luck.
