If you plan to leave Thailand after obtaining your 1 year extension then you will need to get a re-entry permit to maintain the extension.

Do you plan on getting an OA visa purchased in the US or is your goal to get an O visa? There are two different sets of requirements.

You can’t extend your OA visa outside Thailand but you could apply for a new OA visa.

While you can purchase an OA visa while you are in your home country you can also return as a tourist and then apply for a non-O visa inside Thailand. Both eventually lead to a 1 year extension.
