While in Thailand you should carry with you your passport with your visa in it just in case you get stopped for what ever reason.

One or the other. Easier to have 800,000 BHT in your Thai bank account. 2 months before and 3 months after. Minimum balance 400,000 BHT. More work the other way but possible

My wife and I are both 66 good health and we pay 34,000 BHT per year.

Yearly 1,900 BHT to renew retirement visa if you do it yourself. A Visa company will charge a lot more to do for you.

Insurance depending on your age and coverage you want can be 1,100 to 1,200 USD per year and up.

Go to Immigration office yourself. This way you know for sure it is done. Otherwise go to a Visa company but I am sure you will pay more. You do not want a fine or face possible deportation.
