One thing that I was not clear about is that they ask you where you are staying. How would I know that if I have not decided yet? My wife has a cousin in Phuket and we plan on staying there for a while but only after we arrive in Bangkok and stay for a week or 2. So what would be best to list at where I am staying in Thailand?

I have an extremely dumb middle name and it has a "-" in the middle of it and I rarely include it. However, it was the image that messed it up. I am replying and have uploaded my passport pic again and the computer that automatically grabs the info from the picture again missed the "-"
