In my case I used Google translate. Prepared pdf with explain in first page and attached polish and English version. But I apply in Vietnam ;)

Potwierdzam, dokładnie jak jest w paszporcie. W moim przypadku, mam wydany przez "Konsul w Hanoi" i tak też wpisałem. Akcept bez dodatkowych dokumentów w 5-6 dni. Aplikowałem w Hanoi o DTV prawie rok temu.

I confirm, exactly as it appears in the passport. In my case, it was issued by the "Konsul w Hanoi" and that’s exactly how I entered it. Approved without any additional documents in 5–6 days. I applied in Hanoi for DTV almost a year ago.

I'm not Expert, but this don't look like gov website... check for sure

Nice. Any proof of address. I'm in Thailand now, Ed visa to December, next if rules do not change too much - DTV. But I don't know where to apply yet, Poland or somewhere.
