How does the visa application status progress from pending to approval or refusal?

Even though the Los Angeles consulate's website said the usual processing time is about 4 weeks, I went from "pending document check" to "approved" in about 45 minutes. So you never know. Depends on how busy they are and how we'll you've put together your supporting documents.
