Has anyone ever been refused a 30-day extension on a tourist visa in Thailand?

I got the one on the right last time I extended my tourist visa at Pattaya Immigration, Jomtien. I was completely lost as I had never seen that form before. The officer who gave it to me just sent me away puzzled. I then asked another officer, quietly working in a corner, what to do with this. I'm not kidding, she said "Just fill it out yourself". WHAA?! So I wrote my personal info, my address, etc. even though I am not the owner. Submitted that together with the usual documents and got the extension without any issue. Thailand... ;)
