just to be clear, i have never seen this visa.

One date muat be an issue date. The second is the date i need to use this visa by. (Is that 90 days. Or 1 year from the issue date)

So it does not matter what date i enter for the 1st time , after the 1st year ,i need to apply for the next year extension based on the enter before date (not my actual 1st entry using the visa).

I live in ny state.

I sent my passport by priority mail. It was back in my hands with a 4 day turn around including time in the mail. They processed it one day.

I think i remember seeing on the website for ny you can get the visa in one day if you go there. Check the site.
