You don’t need anything like that. The form gives you three lines to put your COVID reason why you can’t go back now. I just paraphrased what the embassy has told another Dutchie, according to Facebook. And we all know Facebook = truth. So, basically what I put there is this: “currently my home country is seeing a surge of COVID and they are in partial lockdown. The Dutch government advices against unnecessary traveling. Also, the Dutch embassy advised its citizens in Thailand to stay there, since Thailand is a safer place.”
