Sorry - I can see how my statement is causing some confusion... I didn't mean to imply that you have to print it out while you're at the embassy. No, you should absolutely print out the form ahead of time and fill it out, or you can use an app to fill out the pdf form then print it out, which was what I did. What I tried to say was that each Thai embassy in a specific country has their own form, which you need to pint out and complete ahead of time to expedite your application process. Cheers!

I made an appointment online to see a specialist for a bunion. I got a confirmation e-mail which I printed out, and that was all I needed. No long-term medical treatment, etc.

I plan to travel fairly frequently, so I'll be out of Thailand prior to the 6 months and from what I understand that should restart the visa for another 6 months when I return. I seriously doubt that the information provided in the application process is part of your visa record, so immigration should not be asking if you're coming back in for the same reason. However, I do not know this for a fact...
