You don’t extend at 90 days , you do immigration reporting every 90 days just like all people stay over 90 days. Can extend for another 180 days. After which you can leave and reenter for another 180 days. Most people just leave and reenter after 180 days .

Alternatively, come and go as you please and receive 180 days each entry

If you don’t get by 11 that’s ok. BUT you must leave the country and re-enter to activate the DTV once it’s approved.
