I called before my appointment to check requirements and was told:

- passport 🙄

- passport copy

- visa copy

- Departure card copy

- 6x4cm visa photo

- T8 form

- COVID insurance to cover extension period

- ASQ certificate

- cash

After arriving at appointment they decided to add:

- Receipt from hotel showing payment of ASQ

- general medical insurance for duration of extension

- apartment lease

- ID card copy of landlord

After returning 2 days later, I was told the lease and ID card were no good because the person signing the lease isn’t the same as person on ID card (it’s a family business); and lease was signed in 2018 and states its for 1 year (attempts to explain it extends automatically each year failed despite the fact the landlord’s receipt for the 2 month deposit are attached to the lease).

Choose counter M50 for Business Visa Extension at CW.

I know this as I'm here doing the same thing right now.
