I have soft power medical in Chiang Mai through Bangkok Hospital for physical therapy.

-doctor’s report with diagnosis or summary, and history of treatment

-treatment plan (mine was 2 months)

-appointment calendar

-receipts showing all appointments are prepaid

I can’t offer much advice other than to say I was approved with that documentation.
