I just came in this morning and bought a cheap outbound ticket for this reason. They literally didn’t ask me anything. Not how long I was staying, where was I going, outbound ticket nothing

Can I apply for the 5-year DTV visa from the USA, and what are the costs involved?

Yes, you can apply from the US. It will take longer (3 weeks to 30 days). If you’re doing it in person you’ll have to find out which location based on state I believe they’re 4. the Royal Thai embassy Washington DC seems to be the main one. You can also do it online.
