P.S. There are some risks getting SETV after working visa. There was reports of refusing of the SETV as well as problems with entering Thailand with SETV after working visa. Using land crossings reduces such risks
I did it in Vientiane lately. All they needed was a package of documents from the school, 2 passport page copies and a visa fee. But there is an online registration for a visit to the embassy and you need to apply it in advance
