what kind of visa fo you hold for that type of Insurance?

Sometimes it does not allow you to add an address other times it does,I have done it twice now and only the second time it allowed me to add an address

Or use and agent,so much easier but if course more expensive, Thai Visa Centre arrange everything including bank statements

it was for my retirement visa i have used them twice never once did they deposit any money into my account,although i my friend did use another agency and they did deposit into his account ,but Thai visa Centre never ask me for that,i did need to send a copy of my book bank to show i have a Thai account, plus my passportvand 2 photos other than that nothing else needed apart from there fee,easy fast and efficient one week max.
