I would never advise risking it, but there are cases when places test using RT-PCR but list only "PCR" on the results. That happened in my case (no reference to RT) and I got in fine just a few days ago.
Again, not recommended, but the real gatekeeper is going to be the gate agent at the port of departure.
Plan on needing a hardcopy. Health Control keeps the PCR test paper after you pass through their station in the immigration process.
I don't think Qatar would accept an image, since they have to highlight the key areas on the form (presumably to show they checked it). I'd advise checking your departing airport and look for places to be able to print (even if you have to buy lounge access).
1. I don't think so. I believe under this scheme, you have to use the transportation lined up through the hotel/program.
2. It was roughly an hour or so for me to get through, but will vary depending on where you are in the queue getting off the plane. Sitting closer to the front will cut your time significantly, as the process moves quickly (just a matter of how many people).
3. You won't have to tell them a time. The hotel should ask for your flight number/details, and they'll be monitoring arrivals.
I don't know about that insurance specifically, but I just sent in the basic coverage letter (1 page outline with names of covered people and general policy info) NOT the actual policy document.
It can be booked under your name, but contact the hotel and ask for a SHABA certificate. They'll then ask for the Names/passport info for the people staying
They'll consider it based on flight time, not application. I wouldn't worry about that. As long as you've been in the US longer than 21 days (i think?) by the time you fly.
