I went through the transfer of stamps to a new passport earlier this year. Very easy exercise. Attached is what you need.

However, there is one thing that is not listed that I was asked for and that was a copy of my bank book together with a copy of it showing that (on the day of visiting Chaengwattana) I had 800,000 baht in my account. So you cannot update your bank book the day before. It has to be on the day of application.
