I got approved in Ankara and indicated the name of the authority. The authority has the name of the country in its name though, maybe it made a difference.

You should be able to apply via the soft power medical route if you want to deliver your baby in Thailand. Contact an hospital, make appointments and ask them for documents. They're usually good at providing documentation.

Contact your bank, it happens often. Recently I could not book a flight on the Japan Airlines website. My bank lifted the restriction after I called them and I was able to book the ticket.

I cannot recall for sure and did not screenshot the pages of my application form. However, I am pretty sure that I entered "xxxxxx Embassy in Beijing" and my visa got approved.
