How can I obtain a Covid-19 recovery certificate in Thailand if I self-isolated without hospitalization?

It's true, I'm currently self isolating myself so this is recent info: you need the pcr test, after which you will be given a phone number to call and those people will figure out the nearest hospital responsible for you incase your symptoms worsen etc. At this point you are given an ID number and are officially in the system. Only this way can you get official recovery certificate...
