Turns out you don't need a form, you just turn up with a letter from your school/work. I just finished doing it and was in and out in 22 mins. Completely hassle free! (Parking's a bitch though)

that link works actually but the form is for a Thai national by the look of it :( I figure I can get a form and fill it in there I'd just rather have it beforehand.
