 : Certificate of Employment + portfolio + bank statement (I gave 3 months just to play safe). This is my experience in Jakarta. I cannot say for other Embassies.

 : Yes, your passport will be kept with the Embassy for 3 days. If you see the visa stamp when you collect it = approved. If no visa stamp = rejected.
