Does PCR-Test result paperwork for entering Thailand with a Thailand Pass need to have passport number on the test result?

I believe it does but my friend says he can’t find that written anywhere on the immigration rules for entry. And he’s saying the place near his house is convenient but doesn’t offer adding a passport number. I told him to suck it up and pay for a private clinic - can anyone settle this debate?
