the extra paperwork for a "based on marriage" Non-O has made many change to the much easier Non-O based on being over 50. However if the OP intends to work, he cannot do so with a Non-O based on being over 50, but he can with a Non-O based on marriage plus a work permit

you surely can work on a marriage visa or extension based on marriage, but this doesn't mean that you can do it without a work permit by the Labour Office. I guess you got one. If not, so have you theoretically been working illegally all the time? I would not hang this subject too high when you apply for a Non-B and its yearly extension

you might actually be health insured on government scheme, by your Thai (?) employer. Ask your employer. Has nothing to do with the enforcement of health insurance for Non-OA visa. You are on Non-B and should enjoy a sound & safe sleep, no insurance needed (but it is always good to have one)

you can, after all, fly back to your home country, which is Sweden, and start the whole process for a Non-Imm-O/A longstay visa, again. I guess you are aware of the new rules regarding the health insurance requirement. Well, you still have the option to apply for a 1-year Multi Entry Non-Imm O in your home country before you wing your way to Thailand again. This one doesn't need the health insurance (although it is wise to have one, just not one of these bogus Thai insurances) In Sweden you can apply for this 1-year Non-O ME, but not in Thailands neighbouring countries

don't attempt to travel to a land border, follow Robert's advice, book a flight and get out of Kansas as fast as can. Fly to Vietnam, Hongkong, Kuala Lumpur, Penang or Singapur or whatever. From out of there you can apply for a tourist visa on any of their Thai consulates

I think you missed to ask your Nong Khai Immigration the correct question. Your question should not be if they "ISSUE" a Non-Imm-O visa, but if they "CONVERT" a 30-day exempt entry or a Tourist Visa into a Non-Imm-O (once your O/A has been made invalid by leaving Thailand without a re-entry permit and doing a simple border run. Then you don't need to visit the Embassy
