For the Test and go, you only need to stay for 1 night at a hotel. Day 5 you can go to a clinic and get tested and submit your forms.

Is the Thai Morchana app mandatory for travelers arriving in Thailand?

It depends what hotel/resort that you stay at. Some request it and some don’t. The only place that asked me was in Koh Chang.
