I think it was about 1500 baht. They took the picture, took copies of all the docs they needed, filled out the forms and went to immigration to get the letter. To me it was worth the money but it can be done cheaper if you are willing to go yourself.

I used an agent for peace of mind as I wanted it done quickly. It is well worth having a Thai Bank account if your planning on spending any time here.

Correct, together with passport and photo and I think they charge a small amount to get it. I used an agent at the time for the residency certificate and I had to give them my passport and rental agreement and they took care of everything for me.

I am on Tourist Visa (STV) and I was able to open an account with Bangkok Bank but they needed a residency certificate before they would open one which I was able to get from immigration.

Not so. Bangkok Bank in Kuad Suan Kaew insist on residency certificate if your on STV and you can get one from immigration as that is what I have done myself so it is not wrong.

I am on STV visa and got a bank account with Bangkok Bank. You need a residency certificate which you can get yourself with immigration but it is probably easier to get it done with a visa agent. Cost about 1500 - 1900 baht. Take this to Bangkok Bank with your passport and they will open an account.for you and you can get an ATM card.

I have used both OS Thai Visa and Assist. I would always opt for Assist just based on personal experience.
