You not need a apointment to extend a visa. Just go to the immigration. There are only a limited number of apointment slots available.

Can not. With visa exempt from the UK you can stay 60 days and extend it once by 30 days. 60 + 30 is 90 days. Then have to leave the country. Sure one can come back and get another visa exempt. But continious staying in Thailand is max 90 days. 3 months

You will need to go to Chengwattana if that is the immigration that issued you current stamp. They need to transfer it to your new passport.
