You re-enter and the 90 day count restarts at 1

State which Embassy/Consulate will be approving it as some take much longer than others so an earlier application might be recommended

Ask your Immigration Office as some allow the letter to be up to a week old whereas others require it to be much less

you were talking about the financial requirements for a subsequent extension not a first extension, man up and accept it

because it was relevant, he cannot already be on a 12 month Extension of Stay without a bank account so those words clearly states his current status, not on an Extension

Where were you married, if outside Thailand have you got the marriage certificate notarized (Embassy), translated and legalised (MTA) and been to your local Amphur to have it registered?

"meet the requirements" means the bank account finances etc.

You apply in person at your local Immigration Office where your TM30 is filed. Go there well in advance and get their list of requirements for the Visa you will be applying for except if it is a Non-B or a Non-ED which requires you to get everything from your employer or school and they will tell you if it can be dne in country or if you have to leave and apply outside.
