The visa is valid for 5 years, multi-entry, you can extend in country once for 180 days, after that you would need to leave and re-enter.

the taxation thing has not happened yet and no-one knows how it will be implemented, but it is 180 days in any year, so leaving and coming back will not 'reset' the 180 days count.

Firstly, 'common law' spouses are not recognised in Thailand. Apply as far in advance as you deem necessary, that will all depend on the turn-around time at the location you apply for the visa.
