and regarding the name - are you sure it's "Starks Wayne Dennis" and not "Starks Dennis Wayne"? the latter is just a tradition of putting the surname first, many countries have that, even in the US you might encounter "STARKS, Dennis Wayne" in some official document. The former I have no explanation for :)
they might not need that much, it might even be detrimental - call them and ask? FWIW, I've talked to the embassy here in Sweden to ask whether it is okay to provide a proof of employment letter and an internal company policy allowing me to work remotely (a 7-page document), and their reaction was like "oh, it's so much text, can't you just add a line "Konstantin is allowed to work remotely" to the letter?". I understand your situation is different, and it's a different embassy, but I reckon it's better to check first.
I've been in contact with the RTE here in Sweden. They want the employment letter/contract to state that I'm allowed to work remotely. the other information I got from them is that the processing times are the same as for TR1 (about a week), and that my wife would need to apply separately after I get my DTV, since she needs to attach my visa to her application.

I wonder if just a regular employment certificate would be enough, even if it doesn't showcase a digital nomad status? The wording is very vague, can be read as either "(contract or portfolio) showcasing status" or "contract or (portfolio showcasing status)".
